Diverter Valves:
Usually need replacing every 5 years or so.
Symptoms:
Hot water only works when the boiler is in heating mode, and running.
Plate Heat Exchanger:
Symptoms:
Fluctuating hot water temperature. Hot/cold/hot/cold etc.
Usually more noticeable in the shower.
Remedy:
Can sometimes be rescued with removal and flushing. Indicates heating system is blocking up boiler.
Replacement may be required, and system filter required for future protection.
Plate Heat Exchanger:
Symptoms:
Fluctuating hot water temperature. Hot/cold/hot/cold etc.
Usually more noticeable in the shower.

Hot water is constant, but only luke warm.
Radiators start warming up when in hot water operation.
Radiators come on in the middle of the night.
